1. Program Description

Voltaire Clinical Research offers a text messaging program ("Program") to individuals who have consented to receive SMS communications related to:

• Appointment reminders and scheduling

• Study visit confirmations and rescheduling

• General study-related updates and reminders

• Requests to complete study-related questionnaires or diaries

• Screening and enrollment follow-up

• Other administrative communications related to your participation in a clinical research study

Text messages are not a substitute for medical advice, diagnosis, or emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. For urgent study-related medical concerns, please contact Voltaire Clinical Research directly at 954-435-9990 or your study coordinator.

2. Consent to Receive Messages

By opting in (including by providing your phone number on an enrollment form, verbal consent, or other means, and confirming via text if applicable), you consent to receive recurring automated text messages from or on behalf of Voltaire Clinical Research at the mobile number provided. Consent to receive text messages is not a condition of participation in any clinical research study, and you may withdraw consent at any time as described below.

You represent that you are the account holder or authorized user for the mobile number provided, and that you are at least 18 years of age (or the age of majority in your jurisdiction).

3. Message Frequency

Message frequency varies depending on your study visit schedule and communication preferences. You may receive multiple messages per week during active study participation.

4. Message and Data Rates

Message and data rates may apply based on your mobile carrier and plan. Voltaire Clinical Research does not charge for participation in this Program, but standard message and data rates from your wireless carrier may apply to messages sent and received.

5. How to Opt Out

You may opt out of receiving text messages at any time by replying STOP to any message you receive from us. After you send "STOP," we will send you a one-time confirmation message that you have been unsubscribed. You will no longer receive SMS messages from this Program. If you wish to re-join, you may sign up again as you did initially, or contact us directly at 954-435-9990

Please note that opting out of SMS communications will not affect your ability to participate in or continue with a clinical research study; alternative methods of communication (phone call, email, or mail) will be used instead.

This Program is available on most major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages. Coverage and message delivery may vary and are not guaranteed.

8. Privacy

Your privacy is important to us. Information collected through this Program, including your mobile phone number, will only be used to facilitate communications related to your study participation and will not be sold, rented, or shared with third parties for marketing purposes.

9. Privacy & Security Notice

Standard SMS text messaging is not a fully secure or encrypted method of communication. Please do not reply to text messages with protected health information (PHI), such as details about your diagnosis, medications, symptoms, or other sensitive medical information. Your information will otherwise be handled in accordance with our Privacy Policy: https://www.voltaireclinicalresearch.com/privacy and, where applicable, our Notice of Privacy Practices for information about how your health information is protected under HIPAA.

10. Disclaimer of Liability

Voltaire Clinical Research is not liable for any delays or failures in the receipt of text messages, as message delivery is subject to effective transmission from your network operator and is outside of our control. We are not responsible for any charges incurred as a result of receiving messages from this Program.

11. Changes to These Terms

We reserve the right to modify or discontinue the Program, or update these Terms, at any time. Any changes will be posted on this page with an updated "Last Updated" date. Your continued participation in the Program following any changes constitutes your acceptance of the revised Terms.
